REHABILITATION PHYSICAL THERAPIST

Schedule:
Relief, day shift 7 am-7:30 pm, various days & weekends

Description:

REHABILITATION PHYSICAL THERAPIST evaluates, plans and provides treatment and rehabilitation as prescribed by a referring physician. Works with family and team members regarding patient's abilities and goals.

1. Maintains an in-depth knowledge of principles, practices, standards and techniques and demonstrates knowledge in accordance with SCMC policies and procedures within pertinent laws and regulations in the following areas:
a. Adheres to Oregon State Physical Therapy Licensing Board.
b. Provides Physical Therapy evaluation and treatment to achieve desired outcomes.
c. Provides staff consultation and training.
d. Provides community resource knowledge and referral service.
e. Capable of providing PT services to other hospital service areas.
f. May be required to drive Rehabilitation Van. If requested, must meet all requirements laid out in Work Instruction.
2. Care Team: Functions as a member of the Interdisciplinary Care Team to assure patient care is accomplished effectively and efficiently in a cost effective manner. Participates in clinical care Conferencing.
3. Documentation: Gathers data and accurately documents information in a timely manner demonstrating compliance with departmental regulations.
4. Patient Education: Participates with team members in facilitating patient's and family's learning throughout the continuum of care. Reinforces patient's continued health care through teaching and/or referral to community agency follow-up.

Requirements:
Education: Bachelor of Science or Masters degree in Physical Therapy from a college or university accredited by the American Physical Therapy Association.Licensure: Must have necessary requisites for licensure as a Physical Therapist by the Oregon State Board of Health. Preferably a member in good standing of the American Physical Therapy Association, licensed or working under provisional permit. Current valid Oregon driver's license required. Current CPR Certification required.

Experience: Desired, but not necessary if requirements are met.

Job Knowledge: Thorough knowledge of therapy procedures, current orthopedic and neurological follow-up procedures, and rehabilitation practices. Must possess a positive attitude and utilize good communication skills. Effective team skills including interaction, support and respect are required.

Computer Literacy: Ability to learn and apply basic computer skills including locating and opening documents, completing forms, opening and sending e-mail messages.
