Sr. Speech & Language Pathologist

  Department: Home Health   Schedule: Part-time, 20 hours per week hours variable as needed, may include weekend rotation. Job

hours may vary due to operation and patient needs between 8 AM and 6 PM and must be available to

work weekends if patient needs requires visits. Current department hours are 8:30 AM to 5:00 PM

 

Pathology. BCLS certification and California S.T..

 

Qualification: Valid CA driver's license, insurance and car available. Minimum of four years S.T.

experience in the past 5 years. Home health experience preferred. Must be competent in dysphasia

treatment. Supervisory skills and experience desirable. Experience in interdepartmental

communication and documentation. Must be able to improvise techniques and equipment in the home

environment. Demonstrated ability to work well with a multi-disciplinary team.

 

Duties: Under the supervision of the Home Health Services' Manager and/or designee, in accordance

with the Physician's Plan of Treatment and all regulations and standards, uses skills and modalities

of professional Speech Pathology: evaluation, assessment, care planning and periodic reassessment of

individual Plans of Treatment to enable homebound patients to obtain optimal levels of independence.

Coordinates services with other team members. Assists with orientation and supervision. Devises

forms and written instructions as needed. Provides inservices for team members. Orders specialized

equipment for patients. Reports patient progress to team members and physicians. Documents skilled

care provided on required forms within specific time frames. Consults with team members and assists

in solving other medical center functions as appropriate. Participates in Quality Assurance and

Utilization Review activites of the department. Other duties as assigned. Job hours may vary due to
