Telephonic Case Management Nurse - (RT6358)

Currently, we have an excellent opportunity available for a Telephonic Case Management Nurse to join our rapidly growing company. This position is located in our Providence, RI office and the job code is RT6358.

Primary responsibilities for this position include:
* Gather relevant data, obtain information from relevant parties, collect information on treatment, and identify barriers and assets to case using national databases
* Set preliminary cost effective and realistic goals from this evaluation
* Work with team to develop a plan (short and long term) optimizing recovery, reducing costs and set timeframes to accomplish this process with specific action items and timeframes
* Perform case management services to move the plan toward medical stability of the injured/sick party, achieving maximum medical improvement and return to work, if feasible and appropriate
* Organize needs, secure information and integrate information into the plan on an on going basis, change approach to coordinate new developments
* Monitor and maintain communication about plan progress with all involved parties, assess at a minimum of two week intervals to update/enhance recovery and coordinate medical and work issues (if appropriate) using Cambridge Best Practices
* Evaluate effectiveness of ongoing activities, amending the plan to reach desired outcomes and if the outcome is guarded, work with the patient, injured worker, and team, seek to achieve stability

Qualified Candidates Will Have:
* AS Nursing or Diploma Graduate with current RN license
* Minimum of five years clinical experience
* Excellent communication skills, both oral and written
* Ability to multi-task
* Good organizational skills
* Demonstrate knowledge and skills in medical field
* Familiarity with Workers Compensation claims a plus
* Strong clinical background
* Working knowledge of URAC standards
* Excellent communication and interpersonal skills
* Strong organizational skills
* Basic computer skills
