Case Manager/Park -- Req. #06-59

TITLE:     Case Manager

 

GRADE: 

 

REQUISITION:  06-59

 

DAYS/HOURS:  40hours per week, to be determined

 

REPORTS TO:  Housing Supervisor

 

DEPARTMENT:  Paul Sullivan Housing

 

LOCATION:  128 Park Street, Boston MA

 

SUMMARY OF THE POSITION:

 

The Case Manager will be responsible for providing services to tenants at Pine Street Inn/ Paul Sullivan houses.  This includes tenant file maintenance, leasing, rent collection, re-certifications and all Housing Authority paperwork. The Manager will work closely with other staff members to ensure that tenants have a clean, safe and secure living environment.  He/she will work closely with tenants in addressing their needs and acting as an advocate with agencies, healthcare and mental health facilities.

 

QUALIFICATIONS:

 

EDUCATION/TRAINING:

 

REQUIRED: 

?  High School diploma or equivalency

?  Counseling experience

?  Good mathematical and calculation skills

?  Basic computer literacy

?  A valid Mass driver?s license in good standing

 

PREFERRED: 

?  A College degree

?  Bilingual-Spanish/English/Creole-French

 KNOWLEDGE/EXPERIENCE:

 

REQUIRED:

?  A desire to work with formerly homeless and indigent individuals

?  Excellent communication and interpersonal skills; knowledge of substance abuse and mental health issues; self motivated, ability to work well with diverse groups of people

 

PREFERRED: 

?  Knowledge of DTA, SSA and other entitlement programs

 

PHYSICAL ABILITIES/SKILLS:

 

REQUIRED:

?  Ability to lift up to 20 lbs.

?  Ability to climb several flights of stairs in the event of an emergency

?  Maintain a safe and sanitary living environment

 



MENTAL ABILITIES/SKILLS:

 

REQUIRED: 

?  Self motivated, open-minded and willing to learn new software applications

?  Ability to diffuse/mediate disputes and deal with stress in a positive manner

?  Good judgment

 

ESSENTIAL FUNCTIONS INCLUDE BUT ARE NOT LIMITED TO:

 

?  Assist tenants in all matters that will help them maintain successful tenancies.

 

?  Assist in the interview of prospective tenants as vacancies become available.

 

?  Interview new tenants within the first week for the purpose of evaluating their service needs, including but not limited to substance abuse, mental health, physical health, employment and benefits.

 

?  Draw up a mutually acceptable and personalized service plan, if needed.

 

?  Evaluate the service plan periodically to make sure that it is current.

 

?  Process tenant income changes and report to proper agencies.

 

?  Promote social interaction and cooperative problem solving.

 

?  Maintain regular, posted office hours to be available to tenants and staff.

 

?  Provide advocacy and liaison services for tenants with appropriate agencies and resources.

 

?  Provide clear, concise and relevant documentation for all meetings and encounters with tenants and with other agencies.

 

?  Meet with tenants who are in arrears with their rent in order to make out a rent repayment plan.

 

?  Be available to provide assistance and consultation in all matters relating to tenancy.

 

?  Maintain files in the Main and Beacon Street offices.

 

?  Collaborate with tenant in filling out Fact Sheet for the PSI database (Service Point).

 

?  Review House Rules and Regulations with the tenant.

 

?  Facilitate communication between Management and the tenants.

 

?  Attend regularly scheduled PSH and PSI staff meetings.

 

?  Be able to respond quickly and appropriately in a crisis situation.

 

?  Meet regularly with supervisor and other Beacon Street staff.
