Case Manager/Service Coordinator -- Req. #06-40

TITLE:   Case manager/Service Coordinator

 

REQUISITION:  06-40

 

DAYS/HOURS:  24 hours; Monday-Friday. (Ability to flex this schedule as needed)

 

REPORTS TO:  Supervisor

 

DEPARTMENT: Paul Sullivan Housing

 

LOCATION:   Scattered Site Housing/Boston - REACH

 

SUMMARY OF POSITION:

 

The person in this position will work closely with both the tenants and the full-time scattered site Case Manager, ensuring all basic needs are met either by PSH staff or outside providers. Additional responsibilities include, but are not limited to ? interviewing prospective tenants, assisting tenants with timely completion of all necessary occupancy paperwork, doing regular individual service plans, maintaining all necessary documentation, assisting with compliance reporting, supporting timely rent collection. This person will be an important member of a team, which has as its goal assisting our tenants to achieve the very highest possible quality of life in shared 2-bedroom apartments in scattered community settings.

 

QUALIFICATIONS:  The person in this position must have a clear commitment to the population we serve and must have a car for transportation to scattered sites.

 

EDUCATION/TRAINING:

 

REQUIRED:  High School diploma or GED, basic computer literacy. Valid Mass. Driver?s License

 

PREFERRED:    Undergraduate Degree.

 

KNOWLEDGE/EXPERIENCE:

 

REQUIRED:  Experience working with a diverse, disenfranchised and challenging population.  Ability to set limits fairly and consistently and maintain good boundaries. Knowledge of effective crisis intervention and respond appropriately in crisis situations.

 

Excellent organizational, communication and interpersonal skills; the ability to multi-task many competing demands.  Ability to prioritize work efficiently and effectively. Ability to work as a team member. 

 

PREFERRED: Experience working with people who have been homeless; knowledge of substance abuse, mental health issues, entitlements, and the Boston area service providers network

 

  Bi-lingual

 

PHYSICAL ABILITIES/SKILLS REQUIRED:

 

Ability to climb many stairs, to lift moderately heavy objects, to work at a computer for prolonged periods of time. Drive to scattered sites as required in all weather conditions.

 

MENTAL ABILITIES/SKILLS REQUIRED:

 

Interpersonal skills, including the ability to engage very vulnerable and disenfranchised individuals; the ability to juggle many competing demands, to prioritize work efficiently and effectively; to maintain good boundaries and to set limits fairly and consistently, to respond appropriately in crisis situations, and to work as a team member.

 

 

ESSENTIAL FUNCTIONS INCLUDE BUT ARE NOT LIMITED TO:

 

?  Fill vacancies in a timely fashion, interviewing prospective tenants, and completing all necessary paperwork as quickly as possible.

 

?  Collaborate with each new tenant to complete necessary database, fact sheet and servicepoint information

 

?  Interview each new tenant within the first week of tenancy, evaluating service needs, including, but not limited to: recovery issues, mental and physical health, employment, benefits, and ADL?s; together drawing up mutually accepted individual service plans, to be periodically evaluated and edited as necessary/appropriate but at least once every 6 months. Maintain weekly progress notes.

 

?  Provide advocacy and liaison services with community agencies and other available and appropriate resources, with the goal of supporting each tenant?s personal growth, enhanced independence and stable tenancy.

 

?  Meet in a timely fashion with tenants who fall behind in rent payments; and provide assistance/support to address timely repayment.

 

?  Support tenants in timely completion of all required paperwork..

 

?  Be available to all tenants, using creativity and perseverance when necessary to engage the most difficult to reach

 

?  Attend regularly scheduled meetings: including supervisory, case management, PSH and PSI staff meetings

 

?  Maintain Servicepoint records

 

?  Maintain all necessary SHP records for annual progress reports, including the Persons Served Worksheets and documentation of service received

There are no supervisory responsibilities included in this job description.

The person in this position will meet regularly with the Program Supervisor
