Job Description:

 

FT position in Allentown office as well as Philadelphia: Assist physically disabled

with independent living goals & determine eligibility for services. Provides direct

services to consumers.  Ensures quality program services for consumers within areas of

responsibility. Perform other duties as assigned. Job # CS-4432

 

Required Education and Experience:

 

Minimum AA obtained in a Human Services field. Two years experience as a case manger conducting service assessments. Additional relevant work experience and/or personal experience accessing services may substitute for educational experience on a one-to-one year basis.

 

Required Skills and Abilities:

?  Knowledge of Microsoft Computer Software (Excel experience a plus)

?  Understanding/Endorsement of Independent Living Philosophy

?  Excellent communication skills (written and oral)

?  Strong organizational skills

?  Experience and understanding of working w/individuals that have multiple disabilities and or challenging behaviors.

 

 Preferred:

 

? HCSIS experience

? Experience accessing, advocating, and or assessing services for people with disabilities
