Intensive Case Manager

Position Summary: The Intensive Case Manager will coordinate formal treatment and support services in order to provide an integrated set of services to the project?s target population.

Duties and Responsibilities:

????????? Maintain a minimum client caseload of twenty-five (25) BHSI and DBH/MRS client populations.

????????? Monitor the quality of treatment through residential and outpatient with assigned caseload.

????????? Provide clinical interventions while in residential treatment and guidance through the aftercare or community phase of treatment to include monthly face to face contact with clients.

????????? Participate in aftercare planning of assigned caseload.

????????? Inform Program Coordinator of any critical or urgent cases.

????????? Maintain communication with Service Managers to provide continuity of care through the BHSI and BHS system.

????????? Attend monthly supervision with Director of ICM and Supervisor of ICM.

????????? Attend all training as assigned by Director of ICM and Supervisor of ICM.

????????? Participate in Team Leader Rotation.

????????? Conduct client follow-up calls and face to face interviews at 30-90-180 days after discharge from ICM unit.

Job Description for: BHSI ICM

Education and Experience:

????????? Bachelor?s degree in a related field (psychology, social work, social sciences, etc)

????????? C.A.C and three (3) years experience in mental healthcare field with significant experience in drug and alcohol services.

?

Essential Functions:? Sitting for a minimum of one (1) hour.? Able to travel via car or bus for off-site visits.
