Job Description:
The Neighborhood Access Coordinator conducts outreach to elders in Southwest Boston according to an approved work plan. The Access Coordinator is responsible for the dissemination of information about Ethos? programs to elders, caregivers, family members, religious organizations, community organizations and referral sources such as hospitals, physician offices and VNAs. S/he participates in the development of materials and the selection of locations for outreach activities, working evenings and weekends as needed. S/he refers elders and caregivers to existing support services. S/he works with local businesses, social service agencies and a committee of elders to develop new community-based activities that address the unmet needs of elders, particularly those that want to age in place. S/he recruits volunteers to help residents with identified needs.
DUTIES:
Be extremely knowledgeable about Ethos and other eldercare resources plus elder/ caregiver issues and concerns.
Represent Ethos in a professional, location-appropriate manner.
Assist in the development of outreach activities at local sites including, but not limited to religious organizations, medical offices, police and fire stations, postal offices, area businesses and social clubs.
Use knowledge gained during site visits to contribute to development of outreach strategies and materials. Prepare Ethos and eldercare information packets for outreach activities. Document all outreach activities including materials distributed, travel and contacts
Coordinate with appropriate Ethos staff to visit selected sites. Distribute literature; talk to staff and participants.
Develop a local advisory committee of elders, businesses, and community leaders to consult on outreach strategies and leverage support for new services and programs.
Visit elders in defined census tracts, complete interviews, and connect elders with services, as appropriate.
Work with elders to design activities to increase socialization and promote community building, such as book clubs. Secure venues with easy access for activities.
Assist with professional evaluation of demonstration project, including documentation of access and delivery process.
Prepare statistics as required.
Job Requirements:
Bachelor?s degree in Human Services, Management or related area, with 1-2 years of experience. Five years of community experience may be substituted for degree.
Demonstrated track record of working with community-based advocacy or health or elder-related organization. Experience in community organizing preferred.
Must be able to write memos, assist in creation of informational materials and use a computerized database.
Experience in marketing, community organizing or a related area.
Outstanding interpersonal, organizational and communication skills.
Physical ability to access sites and transport necessary materials.
Flexible schedule; ability to work evenings and weekends.
Must have transportation to travel to outreach sites in a timely manner.
Intermediate computer skills required.
Bilingual skills a plus (Arabic, Greek, Gaelic, Spanish, Haitian-Creole, etc.)
