Substance Abuse Case Manager
REQUISITION:

DEPARTMENT: Substance Abuse Services/Structured Outpatient Addictions Program
LOCATION: Anchor Inn, North Quincy, MA

SUMMARY OF THE POSITION:
To provide quality substance abuse case management services to homeless adult men according to the program?s established policies and procedures. The case manager takes referrals and conducts screenings for client placement in the Pine Street Inn SOAP program. She/he works closely with the SOAP counselors, PSI direct care staff and case management staff, Anchor Inn program as well as with PSI programs to assist guests in accessing basic emergency services at the Anchor Inn with the goal of engaging them in case management services and assist in the transition to another program or permanent housing. The case manager accurately completes all required record keeping forms, ensures that the client chart is complete and keeps case management progress note up-to-date at all times. She/he works to obtain or maintain credentials and/or licensure by receiving supervision and attending training opportunities. She/he participates in all Quality Management activities including the timely submission of monthly statistics.
QUALIFICATIONS
EDUCATION/TRAINING:
REQUIRED: High school diploma or GED equivalent.
Practical experience in case management and counseling a homeless male clientele with multiple co-presenting wants and needs, including Substance abuse and mental illness, developmental disabilities and housing/economic factors leading to homelessness.
PREFERRED: CAC/CADAC/Certification, training, or formal education
in substance abuse counseling and case management
Bilingual, with an emphasis in English and Spanish.
KNOWLEDGE/EXPERIENCE:
REQUIRED: Minimum two years experience working with a homeless population, providing direct care and/or
active case management and referral services to homeless individuals. Knowledge of the issues related to homelessness, particularly around housing,
Substance abuse, mental illness and economic factors as precipitants to homelessness..A working knowledge of homeless referral networks, with a specific emphasis on transitional programs and housing resources.
PREFERRED: Extensive experience with case management, issues of substance abuse, housing placement, referral networks, housing resources and access to available services for homeless men.
PHYSICAL ABILITIES/SKILLS REQUIRED:Ability to stand for moderate periods of time, stretch, bend, lift
and assist guests up and down stairs during difficult and/or emergency situations. Ability to respond safely and quickly in case of emergency situations. Ability to sit for moderate periods of time at a desk in an office while interacting with clients and/or managing documentation.
MENTAL ABILITIES/SKILLS REQUIRED:Good writing, documentation and computer skills. Ability to perform an array of difficult tasks at the same time. The temperament to work with guests with problems indicative
of untreated emotional/physical health needs and active substance use/abuse/dependency behaviors. Ability to work in intense, emotionally charged situations.
ESSENTIAL FUNCTIONS INCLUDE BUT ARE NOT LIMITED TO:
* Perform client screenings and intakes with identified clients.
* Participate as an active member of the SOAP treatment team, attend daily case conferences.
* Provide referral and case management services to clients, including initial and ongoing development
of individual service plans, referrals and access to appropriate services.
* Coordinate support services and transition to placements secured through the referral process to transitional programs, recovery homes, other supportive housing, and/or permanent housing.
* Manage client folders/files and update progress notes regularly.
* Maintain documentation and statistics to determine effectiveness of case management services.
* Function as a liaison between the SOAP treatment program, Anchor Inn and external agencies who provide outreach services here (i.e. Social Security, housing, legal support).
* Produce required Pine Street Inn program reports in an ongoing and timely manner.
* Perform other program-related duties, as assigned.
SUPERVISION TO BE EXERCISED: (number of direct reports): zero
SUPERVISION TO BE RECEIVED: Weekly individual and group supervision from the clinical director.
