Therapist for Exciting Community Based Delinquency Prevention Program

The Therapist provides therapeutic services to individuals and families, with a focus on assisting those clients in improving aspects of their lives as identified for supportive services. Clients may be referred from various cultural and language orientation families located in the Kensington community of Philadelphia.

Interview clients to gather information about factors which contribute to client?s situation, leading to the need for counseling.

Evaluate client?s abilities, capacities and limitations.

Identify and implement appropriate clinical interventions for assisting the needs of clients, with a focus on supporting client?s own strengths and abilities to improve their own social functioning.

Assist clients in modifying behaviors and attitudes by:

a. Identifying and implementing appropriate clinical modalities;
b. Raising client?s awareness of self and own responsibilities for the creation or perpetuation of identified contributing factors.

As indicated, refer to adjunct community resources and monitor client?s involvement.

Engage and work with families of clients, in an effort to keep them involved with identified goals. Where appropriate an in agreement, do so with clients over the age of 14 years.

Participate in discharge and after-care planning.

Attend regularly scheduled clinical supervisory sessions, prepared to present client cases and clinical issues.

Maintain all client records in a timely and accurate manner. Participate in agency CQI process as it relates to case management.

Attend all staff meetings and scheduled case reviews.

Participate in in-service trainings, professional conferences and professional development workshops.
