Healthcare Services
EOFHR provides:
- Primary Health Care - Primary Health Care provides "medical care for the whole family". The family practice model allows our provider to see individual health care needs in the context of a family's health and environmental circumstances. In many cases, our Primary Care services establish the first ongoing link between the patient and the health care system -- a critical and necessary step in promoting wellness and preventing illness.
- Urgent Care – Urgent Care is
open seven-days-a-week to provide treatment for unscheduled and
non-lifethreatening health care needs. Urgent Care will send patients
with emergency needs to hospital emergency rooms for the treatment
of more acute problems. Urgent Care staff help patients without
a Primary Care Provider to identify a doctor for themselves and
their families. In this way, Urgent Care staff constantly reinforce
the messages of health promotion and illness prevention that lie
at the heart of FHC's family practice model.
- Women's Health Network - The
Health Center has improved delivery of women's health services
in the past year with the purchase of a new, handicap accessible
mammography unit. Please call for more information.
- Advocacy/ Case Management -
Social Services supports the family model of care through the
provision of advocacy, case management, individual and family
counseling, and HIV-related counseling and support services.
- Mental Health - Family Health
Center provides counseling services for individuals, couples,
families, or groups. Referrals can be made by providers or patients,
and psychiatry is available for patients.
- Maternal and Child Health -
Our Maternal and Child Health programs provide prenatal case management
for all pregnant patients, labor and delivery support for patients,
and parenting education.
First Steps
First Steps is intensive weekly home visiting to provide education
and support to pregnant and parenting mothers ages 20 and older.
Healthy Families
Healthy Families is intensive home visiting to provide weekly
education and support to first-time pregnant and parenting mothers
ages 19 and younger.
- Pharmacy - Primary Care Laboratory,
Radiology Services and Pharmacy are vital on-site enhancements
of primary care services. Diagnostic tests and routine X-rays
for both primary and urgent care can be arranged and interpreted
for a wide variety of needs, including blood analysis, HIV testing,
urinalysis, ultrasound exams, and tuberculosis detection and treatment.
